What is the QA Matrix?

Prioritize quality defects by where they were found, severity, cost and recurrence, and track them in the QA Matrix.

The QA Matrix helps your quality team decide which defects to work on first. Each defect is prioritized using where it was caught, how severe it is, how much it costs and how often it recurs, so the team can focus improvement work on the problems that matter most.

Open the QA Matrix

From Home, click the QA Matrix tile under Switch To. The left menu has three pages:

  • QA Dashboard: the list of QA matrices for your facility.

  • QA Settings: admin settings for the QA Matrix.

  • QA Reports: reports on your quality data.

QA Dashboard

The dashboard lists each QA matrix with these columns:

  • Title

  • Facilities: the facilities the matrix applies to, shown as tags (for example a facility name, or a first facility with +1 when there are more).

  • Created By

  • External Defect Range and Internal Defect Range: the date ranges for defects caught by customers (external) and defects caught inside your plant (internal).

  • Actions: open the ⋯ menu for the options available on each item.

Use Search and Filters to find a matrix, and the refresh icon to reload the list.

QA Settings

Click QA Settings to open the QA Matrix Settings. The settings menu has Product Category, Detection Points, Import, Import History, Train Machine Learning and 4M1D Settings.

The Product Category page opens first. It lists each category with its Category, Facility, Date Added, Status (for example Active) and Action (the ⋯ menu).

Click + Add New to add a product category.

QA Reports

The QA Matrix Report page has a Filters button and two tabs: QA Report and FourMoneD (4M1D). The QA Report tab includes charts such as Customer complaints by Product Category, Phenomenon By Priority Index and Classification. A chart shows Report Not Found when there is no data for the selected filters.

Create a QA Matrix item

Click Create New in the top bar and choose QA Matrix.

From defect to improvement

When a defect needs a structured fix, start a Kaizen for it so the root cause and countermeasures are tracked as a project. See Create a Kaizen and add sections.
